Extenuating Circumstances Appeal Form

The following form is to be completed if the student is requesting to be fully dropped or withdrawn from all courses within the current term, after the stated deadline. Appeals will only be considered if a student meets the criteria outlined below which have been deemed "extenuating" and appeals of this nature will not be considered for previous terms.

This form should not be completed by students who misunderstood the drop or withdraw deadlines as stated on the Academic Calendar. It is the student's responsibility to make add, drop and/or withdraw requests each term/session by the stated deadlines.




An extenuating circumstance, for the purpose of this form, is defined as the occurrence of an unforeseen situation or event which was out of the student’s control. Please indicate the category for which your extenuating circumstance pertains (choose all that apply):






NOTE: You may be required to provide any documentation which will support your appeal. This can include, but may not be limited to: medical records, police records, death certificates/obituary, psychiatric history, verification from an unbiased party such as clergy member or school official.

Please provide a detailed explanation of your extenuation circumstance including dates, times and specific information relating to your situation. This is a formal appeal, so proper spelling, capitalization and grammar are required.

 



Your appeal will be reviewed by the Associate Vice President of Student Services within 7-10 business days and you will receive a decision, in writing, via email and paper letter.
