Part-Time Employee Forms (non-students)

This information applies only to PART-TIME EMPLOYEES (non-students); specifically:

•   Adjunct Instructors
•   Graduate Assistants
•   Other part-time temporary employees

All forms are to be completed and returned to Human Resources (Sparks Hall, Room 122) 
prior to employment beginning.  Contact HR at hr@csc.edu or 308-432-6224 with questions.

• Employee Information Sheet
• W-4 ~ (State of NE income taxes will be withheld regardless of your state of residence unless the 9N form is completed and submitted to the CSC Human Resources Office on an annual basis.)
• I-9  (original forms of identification are required. See page 9 of the I-9 for acceptable documents; provide one document from Column A or one from B and one from C).
• Direct Deposit (required) include a voided check 
• Phone Agreement (on-campus employees)
• Drug-Free & Reasonable Cause Acknowledgement
  • Board Policies 5006 & 5009 - informational
• Anti-Harassment Policy
• Confidentiality Agreement
• CSC Employment Application
• Computer Account Request Form

• Key Agreement Form (on-campus employees)
• Criminal Background Check Form  ~ (employment is conditional upon satisfactory background check)

Adjuncts will also need to request official transcripts be sent to CSC Human Resources.
