Please reference the Summary of Benefits document to assist in completing the benefit forms or contact Human Resources with any questions.  Please return all completed forms to Human Resources.

• Employee Information Sheet
• 2013 W-4
• I-9 Form (Original forms of identification are required. See page 9 of I-9 for acceptable documents. Provide one document from Column A or one from B and one from C. Please bring original IDs to Human Resources upon your arrival on campus.)
• Direct Deposit (Required) Include a voided check; full-time employees are paid monthly, on the last business day.

• Flex 125 Plan (Plan Year is September 1, 2012 – August 31, 2013. Optional enrollment)

• Blue Cross Blue Shield Enrollment Form (Coverage begins on the first of the month if you start work on the first.  If not, coverage will begin on the first of the following month. If you begin working on the first of the month, a double deduction will occur in the first payroll as we are required to pay BCBS premiums one month in advance.)
• Salary Reduction Agreement for Insurance
• TIAA-CREF Retirement Enrollment - Mandatory for employees age 30 and above.
• Salary Reduction Agreement for Retirement  or print during online enrollment
